It is a very beautiful poem by Harivanshrai Bachchan .
It consists of a dialogue between a sparrow and her young one.
I had it in school and later I am haunted by it for years and trying to search it madly.

It goes like this

Churungan :- "Chhod Ghosla baahar aaya
Dekhi kaliya dekhe phool
upar uthkar fungi jaani ,niche Jhukke jaana mul
maa ,kya muzko udna aaya ?"
Chidiya :- "Nahi Churungan , tu Bharmaya."

there are some more stanzas and the last one goes :

Churungan:- "
"Main neele agyat gagan ki suntaa hu anivar pukar
Koi andar se kehtaa hain. Ud jaa udta jaa Par maar
Maa ,Kya muzko udna aaya?"
Chidiya:- " Aaj sufal hain tere daine, aaj sufal hain teri kaya"

Like you, I have been desperately searching for it too. What I remember is this:
Chhod ghosla baahar aaya
Dekhi Daalen( I am not sure of this word) dekhe paat
Aur suni jo patte hilmil karte the aapas men baat
Maa, kya muz.....
Nahi.....

Daali se daali par pahuncha
Dekhi kailyan dekhe phool
Upar .... the rest of the verse is as you have written

Kacche-pakke phal pehechaane
Khaye aur giraye kaat
Khaane-gaane ke sab saathi
dekh rahe hain
Meri baat
Maa, kya.....
Nahi....

Then there is, I think one more verse and then the last one tht you have written
